The Brief Addiction Monitor (BAM) is a 17-item questionnaire that measures an individual’s health, alcohol, and drug use.
ITEM CODING
Item 1 is scored on a scale from 0 to 4.
0 = Excellent
1 = Very Good
2 = Good
3 = Fair
4 = Poor
Items 2, 3, 4, 5, 6, 7a-g, 10, 11, 13, and 16 are scored on a scale from 0 to 4.
0 = 0
1 = 1-3
2 = 4-8
3 = 9-15
4 = 16-30
Items 8, 9, 12, 15, and 17 are scored on a scale from 0 to 4 with 17 being reverse coded.
0 = Not at all
1 = Slightly
2 = Moderately
3 = Considerably
4 = Extremely
Item 14 is scored on a 2 point scale.
0 = No
4 = Yes
