The Phobia Questionnaire is a 15-item questionnaire used to measure a person’s avoidance to a particular object or situation as a factor of fear. The PHQ yields one Total Phobia Score and three scale scores, the Agoraphobia score, the Blood-Injury Phobia score, and the Social Phobia score.

The PHQ consists of 15 items that are rated on an 9-point Likert scale as follows:

SCALES
The PHQ yields one total score and three scale scores. Description of the scales and item loadings are listed below. Scale scores are calculated as the sum of respective items. No items are reverse scored.
